If you deny an Account Request, then sign up with the same user name but a different email, the plus cards are created but not the user name card.

http://test.dwagn.org/wagn/JCA+story and http://test.dwagn.org/wagn/JCA+wiki exist but http://test.dwagn.org/wagn/JCA does not.

I strongly suspect this is related to recreate trashed cards when plus card is created, though I'm not sure fixing that will completely fix this... (necessary but not sufficient?)
