restore ability to set type when adding named plus cards+issues

It looks like when the menu to set type on card creation was removed in contexts where it wasn't relevant it was done in an overly general way. A little testing makes it seem like any pre-named plus card creation lacks the menu.

I've actually been wanting to get rid of that interface for a long time.  It's highly problematic and usually very noisy.  And it tends to put a Wagneer's decision in an Editor's face.  As a casual user, seeing "type ... <Basic>" can be really disarming.

I went forward with it last night because I saw how many problems it was causing.  Create permissions used to be type based but now are not.  So with plus cards, where create permission tends to be inherited, type-based options are confusing and usually inappropriate.

My thought is that we could make an explicit way to turn it on (like type:_options), but that this is by far the better default behavior.

add a way to turn on type menu on card creation

If there's a way to distinguish between clicking on a form's +foo and a manually-entered URL it would be nice to keep the old behavior on the latter.
