fix deeply nested forms+todo

testing:

• verify nested forms are working in firefox
• check for formatting issues in as many browsers as possible, especially safari, especially older versions of safari.

post-testing:

• IF there are serious problems with this current code then we'll probably need to convert p's to div's en-masse in existing content.  whew.
