Cardtype menu on new cards:

• restrict showing it to Administrators, or a configurable list of Roles (reduces confusion for non-Wagneers)
• offer a way to make a hierarchical(-ish?) tree/menu of the cardtypes - (the "ish" is John's thought that if we do this, he'd like a cardtype to be able to appear more than once in the tree)
