1. Do you know about the +*self setting? If you add an Evaluation card "foo", when you're ready to lock it you could copy the form from Evaluation+*type+*content to foo+*self+*content. The latter would then override the former, and any changes to Evaluation+*type+*content would no longer affect foo. (Maybe this is what you meant by "up a level"?)
You could alternately use Evaluation+*type+*default instead of +*content. The drawback here is that each time a user edited a card, they would have to click on the "inclusions" subtab. Maybe we want to add setting to default to editing inclusions, but that's not available currently.

Yes, that is what I meant by "up a level". Is there currently a simple way to automate the process of copying the form content from *type to *self? I will most likely limit the user privileges of the end-users, so that they themselves won't be able to modify the card settings.
