dump file of SQL commands to create named card

Advanced:

• a way to toggle including the card history
• a way to toggle including all +*self settings (default on)
• a way to handle non-+*self settings that apply to this card
• for collection, a way to toggle getting the card itself, or all of the cards it references (or both?) - add syntax to specify format is probably relevant
