Ah, I see the issue. You changed permissions on 'The card "Account Request"'. If you look in recent changes, you'll see that card is represented as "Account+*self+*create". What we want is '"Account Request" cards'. That's all the cards with the type "Account Request", and the rule is represented Account+*type+*create".
In simpler terms, when you sign up, you're creating a new Account Request card. To do that, well, you need to have permission to create Account Request cards!
The question, I think, is why going to the options tab of Account Request is not giving you the type option. My first guess is that at some point you changed the type of the Account Request card so that it is no longer a Cardtype card?
You can see and (if necessary) fix it by going to /Account_Request?view=edit_type.
You could also go directly to /Account_Request+*type to see the Set and change the rule.

I was confused because of the description on the config page
" People can request an account via a "Sign Up" link if you allow "Anyone" to create account requests"
the link goes to /card/options/Account_Request and there I turned off "anyone"
