show:menu by default in titled view

{{my card}} = content -> titled

{{mysearch}} = item:closed -> link

{{mypointer}} = item:closed -> link

Includes these, but they're no longer a special case given above:

({{_main}} = open -> titled - but this is no longer a special case)

{{mysearch}} = content -> titled

{{mypointer}} = titled

This will require some tricky migration, and should be prominently mentioned in change notes. --John
