store Pointer items as inclusions+ticket line view
item:link will use {{<cardname>|link}}
for consistency and simplicity (also makes permissions work right regarding when the name is visible)
This will heighten the need for update inclusions when renaming
Remind John to fix http://grouppatternlanguage.org/wagn/captions_per_pattern+*right+*content when this is rolled out.
