update self references upon renaming

In some self-referential scenarios, renaming can get the references table out of date.
+example
To reproduce the bug:
- Rename the "name in flux" card.
- Update reference test inclusions and +search WQL to reflect the "name in flux" card's new name.
- Reload. "reference test" will no longer show up on the search
- Resave the pointer to fix it.
