The ends of headers often end up with an extra br, which makes URLs with anchors unnecessarily ugly.
really, we should strip all html out of table of contents links.

Hard to test new examples, since ToCs don't appear right away (a known problem, though i don't see a ticket for it at table of contents+tickets for item).

Gah. I got it backwards. ToCs have always appeared as soon as there are 4+ headers. When you plus a card to *table of contents = off, they don't turn *off* right away.

Ethan, you mentioned that you also made it so that blank lines wouldn't generate a ToC entry. I tested that and it's fine, unless an h1 or h2 has a space in it. Ticket separately?

I'll have a look first. Might be quick. Problem is html spaces ( ) which should get converted first, then stripped (we don't want to strip all html entities).
